* Driver Commission Type : Percentage / Amount (Fixed)
* Driver Commission : Value of Commission (Percentage / Amount)
* ID Proof : Any Gov. Approved ID Proof Image (jpg/jpeg/png)
* Driving License : Image of Driving License (jpg/jpeg/png)
